"Do you identify as Indigenous, that is, First Nation (North American Indian), Métis or Inuk (Inuit)?"

Looks perfect. Checkbox is fine but if we could have an additional dropdown with multiple choices to allow specificity that would be a bonus. Not sure if that is possible
